Required Forms: 2025-2026 School Year

At the beginning of each school year, parent/guardians are asked to complete the following forms for the 2025-2026 school year by clicking the links below. The electronic forms are closed two weeks after school starts. Students starting school at a later time will be provided these forms by the main office of your child's school.

• Parent Handbook/Code of Conduct/Board Policy/Web-Based Applications Acknowledgment Form

• WSD Publicity Opt Out Form 

• HSA/WEOF Communication Opt Out Form

• Military Opt Out Form   (11th & 12th grade only)
